Yes. The intro text should summarize the procedure that will be used to 
process the suggestion. For the boxes, I would like you to use the exact 
words of N1496 so that we are completely in phase. Or, if you think the 
words in N1496 are bad, I am open to suggestions for a revision.

The submitter's head of delegation.

No. I would like to use simple text for the document. May seem old 
fashioned, but it avoids lots of problems.

That's up to each country. Once a country has submitted the item to WG5, 
the keeper of the repository will assign a number (see N1496).
